Everyone must remember the field trip policy. Students must complete the Student Liability Release Form before any class can be held off-campus. This is not needed for classes that are regularly scheduled elsewhere, such as Community Band.

• Don't forget to offer students the opportunity to purchase insurance for field trips.
• Completed forms are to be turned in to the Director's Office.

This form is to be completed by instructors and students participating in field trips and other off-campus activities. Completed forms should be turned into the Director's Office.

Student Accident Insurance

Insurance is available for purchase through the University of Alaska. Applications must be received in the Risk Management Office prior to the event.
